Please fill out the enclosed order blank listing all of the information requested. The order can be
mailed to us or we will gladly take your order over the phone with either Visa, Mastercard or
Discover Cards.

| SHIPPING & HANDLING |
Our preference is to ship via ground UPS or 1st Class Mail. Please add 6% to the total order, but
not more than $85.00, with a minimum shipping cost of $3.00. Next Day or Second-Day Air UPS
service is available at additional cost. Our handling charge is $3.00 per order. All items are insured
for shipment.
UPS service to Alaska, Hawaii, Puerto Rico and the Virgin Islands requires Second-Day Air rates.
The exact charges will be added in place of our standard 6% for surface shipping.
(The $85.00 maximum does not apply).
All orders shipped outside the USA, either via UPS or Postal Service will have the exact charges
added in place of our standard 6% for surface shipping. (The $85.00 maximum does not apply). |

Due to the extensive packing required to safely ship certain items shown in the catalog, it is necessary
for us to add an extra packing/shipping charge. The items for which this charge is applied are marked
with an asterisk in the price list and the exact charges are noted. |
| GUARANTEE |

We want you to be completely satisfied with your purchase. If for any reason, you wish to return an
item for refund or exchange we will gladly assist. Please contact us within 30 days from the date on
your invoice. If a return is required, we will give you instructions on shipping the item back for
replacement or credit. Any item returned for credit or exchange must be received in re-saleable
condition. Special order items are non- returnable.
